The Well Wisconsin program offers resources to help you lower your health risks and support you in achieving your health and well-being goals.
UW employees, retirees, and their spouses who are currently enrolled in the Wisconsin State Group Health Insurance plan are eligible to participate in the Well Wisconsin program. You can earn an incentive when you complete a health check, a health assessment, and one well-being activity.
The Well Wisconsin program also offers additional well-being resources, including online webinars and more. Create a new account or log in to the Well Wisconsin Portal to get started.

How to Earn your Well Wisconsin Incentive
All required activities must be completed and submitted by the annual deadline. For more information on the Well Wisconsin program, visit the Well Wisconsin Portal or contact the WebMD HelpLine at 800-821-6591 or wellwisconsin@webmd.com.
Step One:Â Complete your health check
Complete a health check by attending an on-site health check hosted by your employer or by visiting your health care provider, completing a dental exam, or completing one coaching session.
Step Two:Â Complete your health assessment
The health assessment can be found on the Well Wisconsin Portal. Request a paper assessment by contacting the HelpLine at 800-821-6591.
Step Three:Â Complete one well-being activity through Well Wisconsin
You can choose from a number of activities that will improve your health and well-being. For a list of well-being activities that qualify for the incentive, visit the Well Wisconsin Portal.

The Well Wisconsin mobile app puts a variety of resources at your fingertips, including the health assessment, well-being activities, and other resources.
