Questionnaires in Job Requisition

Overview

Questionnaires are used to ask specific questions that we would like applicants to answer as part of the job application (i.e. eligibility for work in the US, candidate confidentiality, primary research area for Faculty, etc.). Questionnaires are selected during the create job requisition process.

For a standard (open) recruitment, you must always include the internal and external baseline questionnaires, with no exceptions. This ensures all applicants are asked the required questions needed for compliance:

If needed, you can add a secondary questionnaire for each career site. Questionnaires can differ between internal vs. external career sites when it makes sense to only ask questions of internal vs. external applicants. 

UW-Madison questionnaires

UW-Madison has use cases for eight out of ~45 total questionnaires available in the Workday job requisition. We will not use the other questionnaires because they either request additional application material uploads or are specific to other institutions.

Important: It is best practice to request all application materials be uploaded to the application drop box, which is included automatically in the job posting. This is in part because any application material uploads collected via questionnaire are stored in a different place within Workday than the application materials requested in the drop box.

This means that when posting a job that requires application materials beyond resume and cover letter, you must clearly outline required materials in the Job Qualifications and Additional Details for Job Posting field, under How to Apply, in the job requisition.

Madison's default questionnaire for internal candidates that asks:

  • Please indicate below if you request that your identity be kept confidential. If you do not indicate your preference to remain confidential, the University may be required to disclose your identity and/or application materials. The identity of finalists and successful candidates will be revealed upon request. See Wis. Stat. sec. 19.36(7)

Letters of Recommendation Reference Information

Collects reference information from internal candidates, who cannot be asked via the reference check business process:

  • Please provide the contact information for three professional references. Please include their email and phone number. (Required)

Allows an internal candidate to indicate their interest in a Blue Collar Multi-Shift (BCMS) transfer. 

  • What is the best way to contact you? Please include a phone number with area code and/or email address. (Required)

Madison External Candidate Baseline Questionnaire

Madison's default questionnaire for external candidates that asks:

  • Are you legally eligible to work in the United States?
  • Will you now or in the future require visa sponsorship?
  • Would you like to be contacted about jobs similar to this opportunity at UW-Madison?
  • Please indicate below if you request that your identity be kept confidential. If you do not indicate your preference to remain confidential, the University may be required to disclose your identity and/or application materials. The identity of finalists and successful candidates will be revealed upon request. See Wis. Stat. sec. 19.36(7)
External
Madison Trades Application

Used for Trades job profiles to ask yes/no questions about journey-level status:

  • I have successfully completed an approved apprenticeship in the field that I’m applying for and can provide verification if required to do so.
  • I am currently (or previously) employed as a journey level tradesperson in the field that I’m applying for and am recognized as such by an appropriate authority (e.g., labor union, trade organization, etc.), and can provide verification if required to do so.
  • I claim work experience equivalent to that required for successful completion of an apprenticeship and can provide verification if required to do so.

Used for Facilities Planning & Management (FP&M) custodial applications to ask hours and shift preferences, but available for use by any division:
