Change in Name, Address, or Gender

Overview

Legal Name Change

If you change your legal name, you must update your personal information in Workday. This process differs from changing your preferred name in Workday.

Supporting documentation is required if you legally change your name for reasons other than marriage or divorce. Submit documentation to your  human resources (HR) contact.

This information may include your address, phone number, and/or email address.

Gender Change

Contact the HR office in your school, college, or division for information on how to change or correct gender information, including the documentation required.

Automatic Updates to Benefits Plans

After your name/address update has been processed, the following benefit plans will update automatically:

  • State Group Health Insurance
  • State Group Life Insurance
  • Delta Dental Insurance
  • MetLife Vision Insurance
  • Accident Plan
  • Individual & Family Life Insurance
  • Accidental Death & Dismemberment Insurance
  • Flexible Spending Account
  • Health Savings Account
  • Income Continuation Insurance
  • Wisconsin Retirement System (WRS)
  • Wisconsin Deferred Compensation (WDC) 457 Program

State Group Health

When you change your legal name in Workday, the change will be sent to your State Group Health Insurance provider automatically. You do not need to take any action.

Address Change

You may change health insurance plans and/or health insurance carriers within 30 days of your address change. The change is effective the first of the month on or following receipt of your application.

How to Enroll or Make Changes to Your Benefits

If you change health insurance plan carriers (for example, GHC, Dean, Quartz, etc.), your annual deductible and medical out-of-pocket limits will reset to zero dollars. Your prescription drug annual out-of-pocket limit will not reset.

If your health plan change includes moving to or from an HDHP (High Deductible Health Plan):

  • If you change health insurance plan carriers, all limits will reset (for example, non-HDHP with GHC to HDHP with Dean or HDHP with GHC to non-HDHP with Dean).
  • If you stay with the same health insurance plan carrier, your limits will not reset (for example, non-HDHP with GHC to HDHP with GHC).

Spending & Savings Accounts

Flexible Spending Accounts (FSA)

After you update your name and/or address in Workday, the changes will be sent to the administrator of your FSA. You do not need to take any action.

Health Care FSA

You are not eligible to make any changes to your Health Care FSA election due to a name or address change.

Dependent Day Care FSA

You may change your current Dependent Day Care FSA election within 30 days of your move if a change in residence results in a change to the cost of your dependent care.

Retirement Plans

UW 403(b) Supplemental Retirement Program (SRP)

If you are enrolled with a UW 403(b) SRP provider and need to make changes to your personal information, you must notify them directly.

Complete your address change with TIAA or Fidelity over the phone or in writing. TIAA permits online address changes.

Name changes must be made in writing; contact your recordkeeper for a form.

Wisconsin Deferred Compensation (WDC)

After you update your name and/or address in Workday, the changes will be sent to Wisconsin Deferred Compensation. You do not need to take any action.
