UW–Madison Email Address Request Form

Use of University Email Lists

University of Wisconsin–Madison email lists containing the email addresses of faculty and staff may be made available under the following condition:

  • Mailing lists are to be used only by the individual user and for the purpose specified on the UW–Madison Email Address Request Form and are not to be transferred.
  • Use of the information will comply with all applicable policy and law.

The email addresses will be provided via spreadsheet for the requester to use in a campus platform such as Eloqua or Google Groups.  Any violation of the above condition may result, upon administrative investigation, in revocation of the right to use University mailing requests or in disciplinary action, as appropriate. Questions about the content or appropriateness of specific mailings should be directed to the Office of Human Resources by sending an email to
