Job Summary:
Assists patients in navigating clinics, administrative systems, and patient support services by building working relationships, solving problems, and supporting patients while they learn to self-navigate the health care system to improve coordinated patient care.

Job Responsibilities:
- Works with patients to ensure that care plans established by providers are followed
- Facilitates communication between patients, healthcare teams, and resource providers
- Serves as liaison between various clinical trial programs to evaluate processes and conduct clinical trials on patients
- Schedules and maintains clinical research logistics
- Organizes and maintains appointments for patients
